Public Health Awards

Each year nominations are sought for those who made exceptional contributions to improving the health of Oak Parkers.

These individuals are presented an award named for William Fitzsimmons, a medical consultant to the Oak Park Department of Public Health from 1976-1992. A humanitarian who volunteered his services to the Village, Dr. Fitzsimmons is remembered for his compassion toward others, regardless of their race or socio-economic status, a deep love of medicine and dedication to public service.

Nominations for 2019 Fitzsimmons Awards will be accepted until 5 p.m., Fri., March 22, 2019 - click here to download a nomination form.

Completed forms should be mailed to or dropped off at the Oak Park Department of Public Health, Village Hall, 123 Madison St., Oak Park, IL 60302, Attn: Public Health Awards.

Nominations also may be sent via email to health@oak-park.us. Nominations submitted via email should include responses to all fields listed in the original nomination form.

Award winners are chosen by the Oak Park Board of Health and Health Department
