Death Investigation at Harlem Ave. and Lake St. Published on April 03, 2026 On April 3 at 8:56 a.m., Thomas Cremeens, 48, of Plainfield crashed his Toyota 4Runner on Harlem Ave. at Lake St. after apparently speeding and swerving. The collision involved 4 cars and a Pace bus. Cremeens was found with a self-inflicted gunshot wound and pronounced dead at 9:13 a.m. Fourteen others were injured, none seriously.
