Village conducting community health survey

The Oak Park Public Health Department is conducting a community health survey this spring, and randomly selected Oak Park households will soon be receiving postcards and letters in the mail with information about how to complete the survey.

If you receive these materials, the Village would greatly appreciate your participation.

Survey responses will remain anonymous, confidential and used solely to improve local health services.

Participant feedback and experiences will provide the Village with valuable insights as the Public Health Department works to create priorities and programming for the coming years.

All Oak Park households are invited to participate in the online survey March 19 to April 8. The survey should take about 10-12 minutes to complete the survey and all responses will remain anonymous, confidential and will be used solely to improve local health services. Community feedback and experiences will provide the Village with valuable insights as the Public Health Department works to create priorities and programming for the coming years.
