July 4 Parade Participation Application

The many dimensions of diversity in Oak Park are on display when the annual Independence Day Parade steps off at 10 a.m. on July 4 from Longfellow Park at Ridgeland Avenue and Adams Street. The parade marches north on Ridgeland to Augusta Street, turning east to disperse near Whittier Elementary School at Harvey Avenue.

Please describe the nature of your parade entry, such as float, people walking together, band, horse and buggy, decorated car, bicycles or banners. If your entry is mechanized - car, truck or motorcycle - you must provide proof of insurance. Proof of insurance may be uploaded when this form is submitted, e-mailed to community@oak-park.us, faxed to 708.358.5421 or delivered to Community Relations Department, 123 Madison St., Oak Park, IL 60302. Failure to provide proof of insurance may result in disqualification.

If yes, please make your check payable to the Village of Oak Park and send to Community Relations, Village Hall, 123 Madison St., Oak Park, IL 60302. Please do not send cash.
Throwing candy or any other items along the parade route is prohibited. Do you agree to abide by this rule and inform everyone participating with your organization that any items distributed along the route will be handed out and not thrown?
