Domestic Partnership Registration Declarations of domestic partnership may be registered with the Office of the Village Clerk. The ordinance instituting the declaration and registry process was approved by the Village Board in September 1997. Domestic partners are defined as two unrelated and unmarried persons of the same sex who are age 18 or older, have lived together in Oak Park for at least six months and are jointly responsible for each other's necessities of life. Both persons wishing to make a declaration must be present when the declaration document is submitted to the Village Clerk's Office, where it must be signed under oath in the presence of a Notary Public. Photo identification and proof of Oak Park residency must be presented by each partner at the time of registration. Types of acceptable identification include a passport, drivers license, military identification, resident alien card and naturalization form. Proof of residency includes an apartment lease, mortgage document and utility bill. The fee for registering the declaration of domestic partnership with the Village is $50. The declaration is kept on file at the Village. In addition, each partner is issued a certified copy of his or her domestic partnership declaration at no additional charge.
