Birth, death certificates no longer available from Village As of Jan. 1, 2008, birth and death certificates are no longer available through the Oak Park Department of Public Health in Village Hall, 123 Madison St. The County Clerk is the official record keeper for births and deaths that occur in Chicago and suburban Cook County, but the Village's Health Department had offered the service for births and deaths that occurred in Oak Park as a convenience to residents. However, with Oak Parkers accounting for only about 12 percent of all vital record requests made annually through the Health Department, and costs exceeding revenues from fees, the local service was eliminated beginning in 2008. Cook County offers a range of convenient ways to acquire a birth or death certificate, such as suburban satellite offices, online and through currency exchanges.
